Currently seeking devoted and caring individuals for the following positions:

NURSE CARE COORDINATOR
Position is responsible for working in collaboration with the Nurse Practitioner and the Multidisciplinary Health Education Team in coordinating follow-up for primary
care interventions as well as providing individual and group wellness interventions for adult consumers with serious mental illness and who have identified chronic illnesses or who are at risk for developing chronic illnesses. Performs individual health screenings, completes risk questionnaires, documentation and implementation of
care plan activity, and assists with evaluation of consumer outcomes.

Care coordination responsibility encompasses working directly with consumers to assist in arranging and managing a complex set of resources required to maintain health and function.
